Service Requests
Please follow the five steps below if you are interested in working with CDS.
Step #1
Submit the attached Request for Service form at least 2 months before you will need our service.
Step #2
Susan Weistrop will either e-mail or phone you to let you know if your project fits CDS priorities and schedule. If it does, she will set up a face to face meeting.
Step #3
You will meet with Susan, one of the graduate students who head our service team and, if necessary, resource people. This meeting is usually held at the site of the potential project. A general scope of the project, timeline and costs will be discussed.
Step #4
You will receive a draft of Letter of Understanding from Susan, which contains the scope of work, budget and responsibilities of Client and CDS.
Step #5
Once a Letter has been signed by you and CDS, the graduate student/project manager will contact you to start work

Evaluation of CDS Forms
These evaluations are extremely important feedback for CDS and both to learn from and also fulfill our reporting obligations to the campus. We have two evaluation forms, one that we would like to receive at the completion of the project, “Initial Survey” webInitial+Full Post-hoc Survey108.pdf, and the “Client 6 mo FollowUP Survey” webClient 6mo FollowUp Survey108.pdf, about 6 months later.
