

If you are coming to UWM as an F1 or J1 student, you are required to have adequate health insurance. UWM has negotiated a reasonable rate for international students through a private insurance company.
You have two choices:
Important: If you do not purchase the insurance (#1) or submit an insurance waiver form (#2), you will not be eligible to register for classes in future terms until you submit the information.
Please note: when submitting your or your dependent information, you will be asked for a social security number. If you do not have a social security number, you may use your 990-__-_____ student ID number. Leave the social security number section blank for dependents who do not have a SS#. Also, it is necessary to use a Credit Card that has a BILLING ADDRESS in the United States; if you don't have a U.S.-billed Credit Card you can only enroll in the insurance plan after your arrival in the United States & you have a U.S. address.

Bring evidence of your own adequate health insurance to Garland Hall 138 and complete a health insurance waiver. Be sure to bring your insurance card, as we will take a photocopy for our records.
Students with assistantships should apply for their insurance at the UWM Benefits Office (Engelmann Hall, Room 125) within 30 days of the beginning of their contract, and then bring the proof of enrollment as proof of health insurance. The insurance provided by UWM does not cover medical evacuation and repatriation.
If your own health insurance coverage does not include medical evacuation and repatriation, you can purchase it separately through the Harbour Group, L.L.C. This plan provides benefits for Emergency Medical Evacuation, Repatriation of Remains, and Assistance Services for Individuals engaged in international education activities while traveling outside their home country. This plan is designed to supplement your current medical insurance and does not provide the underlying medical expenses for accidents and /or sickness.
