The University of Wisconsin System Office of Safety and Loss Prevention provides the LATEST policies for workers compensation and latest forms including:

• "Employer's First Report of Injury or Disease" (the WKC-12)
• "Employee's Work Injury and Illness Report."

Also available at this site are the two supplementary reports:
