In this section you can find links and descriptions for all of the forms related to the GIS certificate program or The GIS Council.

Application for Admission

This is the application for admission to the GIS certificate program. Those who are not already admitted to a UWM Graduate degree program must also submit a graduate non-degree application form and an official undergraduate transcript to the Graduate School.

Verify Certificate Completion

This form is used upon completion of the program to begin the process of awarding the certificate.

WebGIS Account Request

Use this form to provide a student with WebGIS access. This form is intended to be filled out by a faculty member.
