Admission to the GIS Certificate Program Form

Click the first text box to begin entering information, and press tab to move to the next field. Click on the SEND button when you are finished. You may print before submitting if you want a copy for your records. Your information will not be saved.

After you have clicked the send button. This form will be emailed to the GIS Certificate Coordinator, Patti Day, for her signature.

Patti Day will submit the form to the Graduate School.

Note: Those who are not already admitted to a UWM Graduate degree program must submit a graduate non-degree application form and an official undergraduate transcript to the Graduate School.
