UW-Milwaukee On-Site Biometric Screenings

Employees and their spouse/domestic partner* enrolled in a Wisconsin state group health insurance plan who complete the Well Wisconsin Incentive Program will receive a $150 wellness incentive from their plan. In support of this program, UWM will be hosting two on-site biometric screenings this Spring.

*Effective January 1, 2016, adult children are no longer eligible.

When: Monday, April 25, 2016
Time: 8:00 a.m. until 1:00 p.m. (½ day)
Location: Ballroom, 1st Floor, Student Union

When: Tuesday, May 3, 2016
Time: 8:00 a.m. until 4:00 p.m. (full day)
Location: Wisconsin Room, 2nd Floor, Student Union


How to complete the Well Wisconsin Incentive Program:

1) Take a Wellness Assessment biometric screening, and
2) Complete your Wisconsin state group health insurance plan's online Health Survey.

• Learn about the Well Wisconsin Program and completing the Health Survey with your state group health insurance plan:

The Wellness Assessment Biometric Screening will measure your height, weight, waist, Body Mass Index, blood pressure, and collect a blood sample by finger-stick to measure your cholesterol (LDL & HDL) and glucose levels.

You may complete your health plan’s 2016 online Health Survey from any computer, at any time (before December 2016). Our Milwaukee area state group health plans have also been invited to provide in-person support during our May 3rd event.

As a reward issued by your health plan, incentives to you/your spouse/domestic partner are considered fringe benefits of employment and treated as taxable income to you.
