Description:

The coordinated service approach, grounded in family centered values and strength-based strategies, is the most effective way to achieve positive outcomes for children and families who have complex needs.  By emphasizing the use of teams, including natural and informal supports, and the development of community resources, families will be assisted in reaching independence from system involvement.

Skills for writing a coordinated single care plan.
