    (Required for all respirator users)

  • For those workers who may be exposed to asbestos above the permissible exposure limit and are included in UWM's medical surveillance program Appendix D, Part II to Sec. 1926 applies and the following forms must be completed:

    • Part 1 of the Appendix contains the Initial Medical Questionnaire, which must be obtained for all new hires who will be covered by the medical surveillance requirements. Initial Medical Questionnaire

    • Part 2 of the Appendix includes the abbreviated Periodical Medical Questionnaire that must be administered to all employees who are provided periodic medical examinations under the medical surveillance provisions of the standard. Periodic Medical Questionnaire
