State the date, time and specific details of your request.  (room numbers, deaf consumer’s name, length of request, type of request, i.e. guest lecture, staff meeting, new student orientation…)

Fee:  $60.00 per hour/per captionist.
Cancellation Policy: We require 48-hours notice (at least two full business days in advance) of a cancellation to avoid any charges.

An assignment over 1 hr and 15 min will require 2 captionists.  We do not bill for captionist travel time for requests on the Kenwood campus.  For off-campus requests, travel time from the Kenwood campus to the captioning location and return are charged at $60.00 per hour.

We will notify you as soon as possible whether or not we can provide captioning service for your request.  If our program is unable to fill your request, we will refer you to community agencies.

If we are able to fill your request, you will need to submit two forms to our office:  Service Request Form and a copy of Form A (Authorization for Payment.)  Both forms can be downloaded below.  The forms can be faxed to our office at 229-6820.  Captionists WILL NOT be sent to the assignment until our office has received the completed forms.
