______ Complete application for Admission to UWM.


______ Make arrangements to take the ACT with accommodations if possible.

______ Submit documentation of disability and LD/ADHD program application to the
             LD Program at the Student Accessibility Center in Mitchell 112.

______ Schedule English and Math or AOC Placement tests with Testing Office (229-4689).
             Request accommodations.


            Complete online Special Accommodations Request Form for placement testing if you need accommodations.

______ Schedule appointment with LD Counselor .
