The Assessment of Emotions

Thank you for agreeing to participate in this study on the assessment of emotions in athletes. Please review the following outline of the study. Those completing all parts of the survey will have their names entered into a drawing to win one of six $25.00 gift certificates at amazon.com.

By clicking on "START" at the bottom of this screen you will be directed to a series of 4 surveys and an informed consent document:

All study materials are accessed through an on-line internet site, and will take approximately 95 minutes to complete.

Before completion of the final survey, you will be directed to an information page. You MUST write down the user name and password on this page. Next, click on the link provided to begin the final survey. At the entry screen for this finaly survey, input the user name and password provided on the previous screen and click "SUBMIT".

At the end of this final survey you will be directed to close your browser. This is the end of the testing.

*REMEMBER: If you must leave the study before completing this final assessment, all of your information will be lost. You will need to restart the entire process to complete the study and be eligible for the gift certificate.
