Office Jacks

Activating a network jack requires a Telephone/Data Service Order (TSO).

A TSO form can also be requested by calling 414-229-5800 or via e-mail to noc@uwm.edu.

NOTE: Data jacks are identified with a "D;" voice jacks are identified with a "V."

  • Jack number (e.g., 2-037D)
  • Computer type: PC or Mac
  • Operating system and version
