YOU HAVE THE RIGHT...

TO DIGNITY -- To be treated with respect and consideration.

TO UNDERSTAND -- To expect adequate explanations and instructions regarding your condtion and care, procedures, medications, tests, and followup care. We urge you to ask questions of your health care provider.

TO CONFIDENTIALITY -- No information from your medical record will be released without your request and signed release. Your privacy will be protected.

IN ADDITION, WE ASK AND EXPECT YOU...

TO BE AN ACTIVE PARTNER IN YOUR HEALTH CARE -- We believe that the choices you make day-to-day will have the biggest impact on your health. We urge you to participate in the decisions about your care and to give informed consent to recommended procedures.

TO BE HONEST -- Give us accurate, current information about your past and present health status, medications, allergies and treatments.

TO BE RESPECTFUL -- Treat our health care professionals with the same respect, consideration, and dignity that you expect from them.
